As a volunteer worker:
1. I will complete assignments to the best of my ability.
2. I will observe all staff rules and policies, accept supervision, and give my supervisor adequate notice before terminating my volunteer services.
3. I agree to grant permission to Eagle Mountain City to reproduce and publicize pictures and/or news articles pertaining to my services to or interest in the Eagle Mountain City Volunteer Program.
4. I understand that in the event I am injured while performing my assignment as a volunteer, I am entitled to Workers Compensation, but for medical benefits only.
5. I understand that the City has the right, should this job assignment or my performance be inappropriate, to suggest alternative placement or terminate my volunteer services.
6. I understand that I will receive no monetary compensation for this volunteer service
Waiver of Liability
I, First Name* Last Name* agree to waive all claims for personal injury or property damage against Eagle Mountain City and to assume full responsibility for volunteer participation with Eagle Mountain City and release the City from any damages or injury which I may sustain working on a volunteer project. However, I am still entitled to indemnification and representation if I am sued for actions performed as a volunteer, as provide in State law and City ordinances.
