Complete this form to rent a fire hydrant meter from Eagle Mountain City.

ATTENTION:
- Do not attach meter directly to hydrant, attach supplied hose to hydrant
- Only use hydrant meter wrench on hydrant, do not use a pipe wrench.
- Use gate valve to throttle not the hydrant
- When closing hydrant, do not over tighten
- Make sure it's not in freezing elements
- Keep stored in a safe place

BILLING:
Each month for the duration of your rental, you will be required to email a picture of your meter read to utilitybilling@eaglemountain.gov by the 15th of the month so you can be billed for your monthly rental fee and water usage.
Hydrant customers that do not submit their read are subject to a $50 no read penalty. Bills are generated by the 10th of the month and delinquent after the last day of the month. Delinquent accounts are subject to a $15 late fee. After three (3) consecutive months of no reads, the meter shall be deemed lost and the deposit forfeited.
